               $89.00 Per routine visit coming to your address includes travel mileage                   for up to 40 miles beyond is $0.75 cents for every mile over 40 miles.

             Mileage cost includes specimen(s) delivery to designated local clinical laboratory              $89.00 Price DOES NOT include Blood Sample Collection or Ekg Tracing

Stat collections are an additional $40 (available 24 hours * 7 days a week )

National Holidays Stat Collections are $150.00 (12:01a - 11:59p)

* We provide an itemized statement at the time of each home visit of all test and services rendered. 

* The statement provided is itemized payment receipt                                                   (payments due at the time of home-visit) 

* The stament provides details your insurance company requires.

* Patient or caregiver must submitt the provided statement directly to your primary or secondary insurance company(s) for reimburstment.
